Application - Step 2

Submission of your application MUST include the following items:

1. Printed and Signed Policy Statement.
2. Completed, Printed and Signed Application.
3. Check for $25.00 non-refundable application fee made out to Jewish Vocational Service.

Application Deadlines:

• Your application for the Spring Semester/Second & Third Quarter grants must be submitted by October 15, 2011.

A personal interview is required for all applicants. Once the application has been submitted, it is the applicants' responsibility to schedule his/her interview.

Interview Deadlines:

• The interview must be scheduled by October 25, 2011 for Spring Semester/Second & Third Quarter year grants.

IMPORTANT NOTE: for the sake of confidentiality of information NO information completed into this form will be maintained on our server and this document does not allow for electronic submission.

The application and Policy statement both require that you have Adobe Acrobat Reader installed on your computer. This is a free program that can be obtained by clicking on the above button. You may save the blank form to your computer and fill it in electronically and then print it out or print a blank form and fill it in manually. You will only be able to save a completed version of the form if you have a full copy of Adobe Acrobat. Once complete, the application must be manually signed and returned to the Hilb Administrator, Jewish Vocational Service, 4300 Rossplain Road, Cincinnati, OH 45236 by the application deadline, stated above.
